Chronological History
The following is a list of chronological events which can be verified via the court records. After meeting the captain, CFS was arrested in multiple counties and states for seven straight consecutive years and charged with offenses against women, in stark contrast to the stated CFS mission of defending vulnerable women from men who target those suffering from illicit drug use in exchange for sexual favors. The events include an attempts to orchestrate a Jordan Neely situation where Jack Terry would have been attacked by passengers in a Los Angeles subway station (video).
July 24, 2016: CFS BlackBook met an army captain stationed at Ft. Rucker, Alabama replied to a CFS Craigslist personals ad
August 1, 2016: a business license for CFS BlackBook was filed (Dothan, Alabama)
October 2016: CFS was arrested in Dothan, Alabama on allegations of domestic violence (Sidebar: Marion Nicole Clyburn)
October 13, 2016: charged with weapons out of Gulfport, MS
the charges are almost ten years old and should have been dismissed failure to prosecute as a matter of law. The indictment returned on June 27, 2016.
October 9, 2017: CFS was arrested in South Plainfield, New Jersey on allegations of domestic violence involving Monique Thaxton (Sidebar: Middlesex County).
October - December 2017: While confined at the Middlesex County jail, CFS was visited by two men who identified themselves as Homeland Security agents and who offered CFS an opportunity to serve as a paid informant to help locate terrorists within the United States. The names and credentials of these two visitors, as well as the exact times of their visits, can be verified through the Middlesex County jail visitors log book and by reviewing the facility's CCTV video recordings.
December 12, 2017: CFS entered a plea bargain for five years probation in exchange for a guilty plea to terroristic threats, a third-degree crime punishable by up to five years in prison. The original allegations were that Monique Thaxton was beaten at knife point and held hostage for more than three hours inside a private school while classes were in session in South Plainfield. However, CCTV video from the school proved those allegations to be fabricated and false. (Assistant prosecutor Keith L. Abrams; Lindsay Gargano, appointed counsel).
December 27, 2018: CFS was extradited from Phoenix, Arizona to New Brunswick, New Jersey
June 19, 2019: Jack Terry was sentenced to serve 364 days in the Middlesex County jail for violating probation by failing to perform as a paid informant under the “cooperation agreement.” Despite his status as an ex‑offender, the 364‑day sentence reflects that Terry was treated as a “first‑time” offender for this violation
November 23, 2019: Las Vegas, NV - an attempt to engage Jack Terry in terrorist rhetoric while in a Las Vegas masjid was reported by Jack Terry to the FBI via its tip line website, which resulted in the arrest of a military contractor who held Homeland Security clearances. The Muslim place of worship was Masjid Sabur. (Sidebar: Las Vegas)
November 9, 2020: charged in Pleasantville, NJ for domestic violence involving Monique Thaxton. First video evidence of the actual use of the technology for violence (Sidebar: Under Armor)
Sentence: five years probation in exchange for a guilty plea to attempted aggravated assault.
January 27, 2023: sentenced to serve three years state prison for violation of probation
June 29, 2023: resentenced to “time served” and released from Bayside State prison, Leesburg, NJ
The week of August 29, 2023: Jack Terry appears in the Atlantic County courthouse (Judge Delury) in support of Monique Thaxton, who is then permitted by the court to enter a guilty plea at arraignment on the indictment and was released from the Atlantic county jail
September 15, 2023 - Jack Terry arrested in Galloway, NJ for domestic violence involving Monique Thaxton
Sentence: August 28, 2024 - 231 days in jail for unlawful possession of a weapon. The allegations were that Jack Terry, on September 15, 2024, beat Monique Thaxton at knife point an held her hostage for an unknown period of time
August 3, 2025: Jack Terry receives a cellphone call from whom he believes to be Ibn Hassan, requesting that Jack Terry retrieve Ibn Hassan’s personal property from the Union county jail and a BMW X5 from a parking garage near the Union county jail
August 4, 2025: Jack Terry gets into a motor vehicle accident with the BMW X5 after leaving the Union County jail parking garage. The responding Elizabeth police officer failed to report the motor vehicle accident. August 19, 2025 - The Toms River police department arrests and involuntarily commits Jack Terry to the Barnabus Behavioral Health mental health facility on the the allegation that he posed a threat to self proclaimed reverend Donnie Clyburn of the Path of Life Ministries church in Toms River, NJ. Donnie Clyburn is a known child molester who raped his cousin Christina Rhines when she was 12 and fathered two children with her before she was 15 years old (Sidebar: Ocean County)
October 11, 2025: arrested in Santa Monica, California for vandalism to a leased vehicle (Sidebar: Santa Monica)
November 21, 2025: a Venice Beach library security guard steals terry’s pouch containing his driver’s license, debit cards, etc.
January 3, 2026: arrested in Venice Beach, California for vandalism to a leased vehicle (Sidebar: Los Angeles)
The week of January 26, 2026: Jack Terry reported an attempt to make it appear as if Jack Terry was planning to bomb the Wilshire Western train station in Hollywood, CA via the Homeland Security tip line website (Sidebar: Wilshire Western)
January 28, 2026: Jack Terry recorded an attempt by a court psychiatrist to determine CFS was incompetent to stand trial (Sidebar: Incompetent)
February 3, 2026: Jack Terry arrested by the Los Angeles police department, Pacific Division, and involuntarily committed to the mental health ward at Southern California hospital under the name “Allen Terry”, where surgery to remove a tracking chip from the left side of Jack Terry’s back was performed
March 3, 2026: Jack Terry arrested and charged with aggravated assault with a deadly weapon with grave bodily injury
Sentence: May 7, 2026: charged with two counts of vandalizing leased vehicles; and a reduced charge of attempted aggravated assault (swung at at someone and missed) Jack Terry is released after 66 days in jail
July 10 -16, 2026: The Toms River police department arrests and involuntarily commits Jack Terry to the Barnabus Behavioral Health mental health facility on the allegation that Jack Terry sent emails to a South Toms River detective (Jack Sayegh) that were “concerning” and that Jack Terry again poses a threat to self proclaimed reverend Donnie Clyburn. While there, Jack Terry is forced medicated and received three injections of an unknown substance in both legs and his left foot (Sidebar: Under Armor)
